A 100% drug tariff lands right where the Big Three just bet big

The week's heftiest development for the channel is a deadline: on September 29, a 100% tariff on patented pharmaceuticals and their ingredients hits every importer not already covered. It lands squarely on the healthcare distributors who have spent the last two years extending their grip on the specialty drug channel. This issue pairs the policy with a spotlight on the vertical it most affects — if you touch pharma distribution in any way, this one's worth reading closely.

A 100% tariff on patented drugs takes effect September 29

Under an April proclamation, Section 232 tariffs of 100% apply to imported patented pharmaceuticals — branded drugs in the FDA's Orange and Purple Books — along with their active ingredients and key starting materials. Seventeen named innovator companies were hit July 31; everyone else in scope faces the duty on September 29. The structure is tiered: 0% for companies that sign both US pricing and onshoring agreements, 20% for onshoring plans alone, and 15% for the EU, Japan, South Korea, and Switzerland. Generics and biosimilars are exempt for now. The Commerce investigation behind it found the US makes only about 15% of its active pharmaceutical ingredients domestically — which is exactly the dependency the tariff is meant to force onshore.

Sector Spotlight: Healthcare & Pharma Distribution

This week's vertical under the lens

The Structure

The Big Three run the drug channel — and keep extending it

US pharmaceutical distribution is one of the most concentrated channels in all of commerce: McKesson, Cencora, and Cardinal Health move more than 90% of the country’s prescription drugs, on combined revenue near $900 billion. And they haven’t stood still. Over recent years the three have spent well over $16 billion acquiring the management-services organizations behind physician practices in oncology, gastroenterology, urology, and ophthalmology — forward-integrating from pure distribution into ownership of the specialty channels where the highest-value, buy-and-bill drugs flow.

The Collision

Why the pharma tariff hits hardest where the Big Three just invested

The tariff and the consolidation are on a collision course. The 100% duty targets patented, branded drugs and their APIs — precisely the specialty and oncology products the Big Three have been building their growth around, and precisely the imports the US most depends on. For distributors, that turns sourcing and onshoring strategy into a channel-defining problem almost overnight: which suppliers hold company-specific agreements, how quickly APIs can be reshored, and how landed-cost changes ripple through buy-and-bill economics to hospitals and clinics. Expect origin, agreement status, and real-time landed cost to become first-class data in healthcare procurement — the same data-integrity theme now playing out across every B2B vertical, arriving in pharma under a hard deadline.
